The notebook belongs to Guri, an angel and cupid who accidentally writes down Seiji Aino, a regular high school student. Unless he kisses someone within 24 hours, Guri will die and Seiji will remain a virgin for life. Guri convinces Seiji to kiss his crush, Akane Hiyama, who turns out to have obsessive and psychotic feelings for him. Guri later adds her own name to the Kiss Note, creating a polyamorous relationship.

Seiji is a levelheaded individual trying to please the women around him. The main characters include Akane, a violently obsessive class idol; Yuzu Kichougasaki, Akane's shy half-sister who is in love with Akane; and Shikimi Shiramine, a sadistic and manipulative cousin of Akane and Yuzu. All except Guri are minor angels, made immortal through the Kiss Note. Guri's father, God, asks Seiji to help Guri search for the meaning of love, as understanding love is a requirement for becoming God. In the end of the manga, Seiji marries Akane, Yuzu, and Guri in a wedding ceremony.
